Tribal Need Financial Analysis?

I recently received an e-mail from my university's financial aid department asking me to fill out and fax them the 'Tribal Need Financial Analysis' form. I applied to the school as a Native American/Pacific Islander - which I am - but the form asks for the name and addresses of four tribal agencies and I have no idea what this is. I am Native American, but I never lived on any reservation or anything and don't know what a tribal agency is let alone am able to provide this information. Is this form necessary? I don't recall ever applying to any racial based scholarships, but I'm afraid not submitting this form could interfere with receiving already awarded grants and loans. Any help or advice?
